Improving Anticoagulation Safety in Pediatrics

Rady Children’s Hospital in San Diego will deploy this grant to develop safer anticoagulation therapy in the pediatric population. The team includes physicians, pharmacists, nurses and lab personnel from both inpatient and ambulatory practices. We will work primarily to prevent patient harm by identifying those patients who would benefit from prophylaxis against thromboembolism and by creating treatment protocols, clinical pathways and multidisciplinary team processes for prophylaxis and therapeutic anticoagulation. These protocols will include ordering, monitoring and titrating processes as well as methods for early detection and pathways to mitigate harm should an adverse event occur.
