Early Predictor of Critical Health Status (EPOCH) screening tool

An easy-to-use screening tool, called Early Predictor of Critical Health Status (EPOCH), will be developed for the purpose of early prediction of the deteriorating condition of a patient. The EPOCH tool will aid the nurse in early recognition of subtle, yet significant, change in the patient’s condition. Eventually, this rapid assessment screening tool will be incorporated house-wide as part of the routine nursing assessment process as well as into nursing education. EPOCH will serve as a mechanism to alert the nurse of the potential for imminent clinical demise. Early recognition of significant change will offer valuable time for additional assessment, interventions, and for consultation with the Rapid Response Team before the patient’s condition becomes critical. Our measure of success is to decrease the number of codes and adverse effects due to Failure to Rescue.
